On Donation
of 2 Percent of Income Tax
In the name of the animals
we shelter we are asking to support the activities of Lesė by
donating the 2 percent of your income tax until the 1st of May.
This contribution, which costs you nothing, is the main source of funding
for our organization and is used for animal rescue and sterilization. In the year 2009 we treated and
rehomed more than 450 animals and fully or partially financed
sterilization of more than 600 animals
using the proceeds from donations.
You can donate
the 2 percent of the income tax in the following two ways:
The
first way: delivering a filled-in FR0512 form to the State Tax Inspectorate.
- Print-out the FR0512 form.
- Using capital letters
fill-in your first name, last name, national identification No., place
of residence, e-mail address, telephone, the date of completion and
the tax period for the year 2009 and necessary requisites of Lesė
at the top of the form:

- At the bottom of
the form put your signature and provide your first and last names.
- Put the form into
an envelope, seal it and sign over the seal.
- Bring the sealed
envelope to the State Tax Inspectorate of your town until the 1st of
May.
The second way: filling-in
the FR0512 form online (if you use e-banking).
- Login to your e-banking
account and select electronic declaration. The bank will open the site
of the State Tax Inspectorate for you.
- Open and fill-in
the FR0512, v.1 form. All the instructions are explained in detail.
Requisites of Lesė are provided below.
Important:
- If you are filling-in
an annual income tax declaration, the application for 2 percent of the
income tax should be submitted along with it.
- 2 percent of the
income tax can be allocated to several organizations. They must be indicated
in the same application form.
- If your application
contains mistakes, the State Tax Inspectorate will return the form for
correction and you will be able to fill-in a revised form until the
1st of July of this year.
